Ways to Reach Us

If you are already in an active program, your care coordinator is reachable directly rather than through this form. If you are reaching out for the first time, the assessment is the better starting point, and it requires no commitment beyond a willingness to look at the data.

Send a Message

Tell us what is going on.

The more context you give, the more useful the reply. There is no wrong amount of detail here, and nothing you write goes anywhere except to our team.

Submitting this form does not create a physician-patient relationship. Please do not include sensitive medical details here. Those are discussed privately at your assessment. See our HIPAA Notice.

Get Directions
Valet

Valet parking is available at the main entrance. Leave the keys and walk in. Nothing to arrange in advance, nothing to settle on the way out.

Private Entrance

A separate arrival lane is available for Valerian patients who require additional discretion. Mention it when your appointment is confirmed and it is arranged without further conversation.

What to Expect

No waiting room and no reception queue. You are met on arrival and taken directly to a private suite. Appointments are scheduled so that arrivals do not overlap.

Before You Reach Out

The things people ask
before they get in touch.

If you already know you want to begin, go straight to the assessment. It is the actual entry point and skipping the form saves you a step. Use this form if you have a question first, if you are unsure whether any of this applies to your situation, or if you want to understand the range before committing to an appointment. Both reach the same team.

Within one business day, and usually sooner. Inquiries arriving over a weekend are answered on Monday. If your question is time-sensitive, calling during business hours is faster than the form: Monday through Friday, 8:00 am to 5:00 pm Central, and Saturday until 3:00 pm.

Enough that we can give you a useful reply rather than a generic one: what changed, roughly when, what you have already tried, and what you are hoping to get back. That said, please keep sensitive medical specifics out of the form. Those belong in the assessment, where the conversation is private and covered by our HIPAA privacy practices.

You are not expected to know, and figuring that out is the physician's job rather than yours. Programs here are built from your panel, so there is no menu to choose from and no decision for you to get right on your own. If you want to understand the range before reaching out, the programs page covers all eight, but reading it is optional.

No. Valere operates as a private-pay practice, which is what allows the panel to be as comprehensive as it is and the appointments to run sixty to ninety minutes. Payment plans and membership options are available, and cost is discussed after your results are reviewed rather than quoted in advance of them.

Yes. Valere operates with full HIPAA compliance, and your clinical information is never shared without explicit consent. Many Valerian patients here are public-facing professionals for whom confidentiality is a requirement rather than a preference: private arrival, private suites, no shared waiting areas. Our full Notice of Privacy Practices is on the site.

Or skip the form
and start with the data.

Your assessment begins with the bloodwork that shows what is actually happening. Everything after that is built around what it shows.
